Cook County (Not Chicago):
Opt-Out Cities: If your City has opted out of the Cook County Paid Employee Sick Leave requirement, then the Illinois Paid Leave for All is required. Check all boxes in the Cook County Paid Employee Sick Leave section and proceed with the Illinois Paid Leave process.
Other Cities: If your City has NOT opted out of the Cook County Paid Employee Sick Leave requirement, then the Cook County Paid Employee Sick Leave is required. Check all boxes in the Illinois Paid Leave for All section and proceed with the Cook County Paid Employee Sick Leave process.
